Full Circle Health Network is an integrated network of nonprofit providers delivering coordinated, community-based services to vulnerable children, individuals and families across California. Full Circle exists to ensure more Californians can access equitable, culturally congruent, and trauma-informed care from a high-quality network of community-based organizations that address their whole-person and whole-family needs. Achieving that goal requires new paradigms for contracting. Under a new initiative with funding from the Office of Youth and Community Restoration (OYCR), Full Circle aims to create a meaningful bridge between Medi-Cal Managed Care Plans (MCPs) and community-based organizations (CBOs) serving youth involved in the justice system.
About this Position
Under the direction from the Assistant Director of Justice Programs, the Justice Provider Network Liaison will be responsible for developing a statewide network of providers serving justice-involved youth to contract with Full Circle to deliver CalAIM services. This an exciting opportunity to be at the nexus of California’s CalAIM Medi-Cal reforms to help make the systems work better for at-risk or justice-involved youth. This position will be part of a team aiming to for having 1,000 at-risk or justice-involved youth served through managed-funded CalAIM services by the end of 2025.
Responsibilities
Identify providers to target for recruitment to bring into the justice serving network by sourcing referrals from key partners, including but not limited to, the CA Office of Youth and Community Restoration, Sierra Health Foundation, Public Works Alliance, and the California Alliance for Child and Family Serving Agencies.
Support providers in meeting the requirements to contract with Full Circle and be eligible to participate in MCP networks for specific services by providing technical assistance support, answering questions and developing key communication materials.
Developing content for tailored trainings to support providers in meeting MCP requirements, such as getting an NPI, enrolling in Medi-Cal, recruiting staff, and developing policies and procedures.
Developing and supporting an ongoing learning collaborative/community of practice among participating providers.
Serve as Full Circle’s primary point of contact to justice serving providers to ensure they receive the technical assistance necessary to successfully contract and serve justice-involved youth.
Be a subject matter expert of Enhanced Care Management specifically in the Justice Involved (JI) Population of Focus (PoF) including children and youth, pre-release services, understanding of Medi-Cal benefits and the concept of embedded/in-reach provider services and re-entry in the community.
Ensure that Full Circle and providers are meeting all contractual obligations, and developing and overseeing corrective action plans in instances where gaps are identified.

Ability to work independently and maintain positive relationships with internal and external contacts at all levels.
Ability to communicate and work effectively with a variety of providers and individuals with demonstrating problem-solving skills.
Must be able to represent Full Circle in a positive and collaborative manner externally to relevant county departments, correctional facilities, community partners and other key stakeholders.
Managed care or network management experience is a plus.

Education and Experience:
3 - 5 years of experience working with programs or organizations focused on serving justice-involved individuals, with a preference for youth programs.
